| Physical Therapist Exempt Status Position Hours Work Location Duration Employee 40 hr work week Chicago land area Full-time HOME HEALTH AGENCY SUMMARY Privately owned Home Health Agency Provider opening new facilities in the Chicago land area to accommodate rapid growth and expansion. We are a recognized Agency providing best in class service in the nations’ healthcare rehabilitation industry. We are expanding our facilities and have 25 positions available for immediate hire. Our vision is to be recognized as the industry leader that provides best in class services to our patients and our clients. Our goal is to provide extraordinary customer care and continuous quality treatment to our patients and their families. KEY ACCOUNTABILITIES ● Organize and participate in rehabilitation programs that improve patient mobility and alleviate pain ● Ability to assess, analyze and plan treatment for patient care ● Familiar with workplace knowledge and basic physical therapy methods and techniques ● Development of treatment plans that increase strength and decrease, or prevent deformity ● Ability to work effectively within a Home Health Care environment EDUCATION AND EXPERIENCE ● Licensed Physical Therapist ● 1-2 years experience preferred; will train recent college graduate with strong skill set ● Strong writing and communication skills ● Strong advocate for patient rehabilitation and support ● Works well within a team environment ● Pro-active work ethic requiring minimal supervision Salary $50,000.00 plus benefits, travel & expense reimbursements Contact: Sonrise International Fax: 630.701.2320.
